Swiss authorities require these documents for a VAD.

First Step to Apply for VAD:

To begin your application, indicate you are ready for the procedure. (see Procedure)

For this you will need to have the following documents ready:

Your most recent medical records

A scanned copy or clear photograph of your passport

Amongst others, the following documents are required a bit later from everybody regardless of marital status:

Passport (or ID used to cross the border into Switzerland).Please make sure you uploaded or e-mailed a scan or good photo thereof and bring the original to the VAD.

Proof of residence(Copy the first sheet of your tax return with your current full address on it.)

Your long-form birth certificate(with parents' full names, usually obtained from a government Registry Office)Must be re-issued in the last 6 months prior to your VAD date
