Documents Required

A range of documents is required by the Swiss authorities in order for a VAD to go ahead. While copies of these documents can be uploaded as part of the online application process, the originals must be brought in person to Switzerland.

Passport (or ID used to cross the border into Switzerland)

  • Please make sure you uploaded or e-mailed a scan or good photo thereof.
  • Bring the original with you to your VAD (Voluntary Assisted Dying)

Proof of residence
(Rates notice, utilities bill, etc.)

 

If married:

Your long-form birth certificate
(with parents’ full names, usually obtained from a government Registry Office)

  • Must be re-issued in the last 6 months prior to your VAD date
  • Must be in English, German, French, Italian or translated to one of these languages

Marriage certificate
(usually obtained from a government Registry Office)

  • Must be  re-issued in the last 6 months prior to your VAD date
  • Must be in English, German, French, Italian or translated to one of these languages

Spouse’s birth certificate
(Long form with parent’s names on it)

  • Must be re-issued in the last 6 months prior to your VAD date
  • Must be in English, German, French, Italian or translated to one of these languages

 

If single:

Your long-form birth certificate
(with parents’ full names, usually obtained from a government Registry Office)

  • Must be re-issued in the last 6 months prior to your VAD date
  • Must be in English, German, French, Italian or translated to one of these languages

Statutory Declaration

  • The declaration must have the exact wording: “I solemnly declare that I am single and have never been married.”
  • This declaration must contain your full name, together with the full names of your parents, your date of birth and current residential address.
  • Signed and dated within 6 months prior to your VAD.

 

If widowed:

Your long-form birth certificate
(with parents’ full names, usually obtained from a government Registry Office)

  • Must be re-issued in the last 6 months prior to your VAD date
  • Must be in English, German, French, Italian or translated to one of these languages

Death certificate of your spouse (a copy is sufficient)

 

If divorced:

Your long-form birth certificate
(with parents’ full names, usually obtained from a government Registry Office)

  • Must be re-issued in the last 6 months prior to your VAD date
  • Must be in English, German, French, Italian or translated to one of these languages

Divorce certificate (a copy is sufficient)
